Case register 5

Table of cases 1333-3000 (19 May 1992-8 August 1997), largely in chronological order.

Capturing name, disease, date contacted SPAID, solicitor (name and date contacted), SMB (Special Medical Board) decision awarded/refused, notice to appeal given (ACK), appeal papers received, SPAID's submission sent, hearing date, comments. Of these, columns chiefly used are the first 3-4 and the last.

Case register 6

Table of cases 3001-3735 (11 August 1997-13 March 2007), largely in chronological order.

Capturing name, disease, date contacted OEDA, solicitor (name and date contacted), SMB (Special Medical Board) decision awarded/refused, comments.

This schedule marks a departure from previous practice, in that columns that remained empty in past registers have been removed. Also, the third column now bears the new name of the organisation.

Case register arranged alphabetically by surname with columns for name/address/disease. In address field, date of SPAID-contact. No case numbers.

Further includes a ‘Special interest’ section containing:

  • list of British ships (25 October 1994), alphabetized by last name
  • (partial) list (case-numbered) of ?mesothelioma? cases, with information on settlement, press coverage; some aggregated by ‘short exposures’, ‘special interest’
  • press items, list of MRSA sufferers, list lung cancer cases, bronchiectasis, cryptogenic fibrosing alveolitis etc.

  • list of overseas cases 1988-2007, in chronological order
  • medical reports by Dr R M Rudd, 2000

Database printout, July 2003

Printout of the OEDA case database in the form of a table, 28 July 2003. 171 pp, listing cases 2-3650 (with gaps) and cases 4001-10,000 (with extensive gaps), followed by a few rows with anomalous case numbers. This and other indicators suggest that there may have been a technical problem with the database.

The printout appears to have been generated to support OEDA's case for continued funding from Association of London Government (ALG; formerly the London Boroughs Grant Unit, LBGU), the organisation's main source of funding since 1983.

  • note from Nancy Tait proposing to use this printout, rather than the electronic database, to record important updates henceforth
  • overview of codes and what each column in the database is intended for
Cases by number, 43-3730

Set of case summary sheets, some of them print-outs of electronic case sheets, others manually populated photocopies of database entry mask. In reverse numerical order, counting backwards from 3730 to 43.

There are occasional gaps throughout and considerable gaps c.2200-43. Annotations suggest that some cases may have been excluded on the grounds that the database was reserved for cases of asbestos-related diseases.

Set of manually filled out forms summarising case information, presumably generated for database entry. In alphabetical order.

  • smoker from/to, ex smoker from/to, never smoked

'Spaidbas'

Instructions on how to use the computer in the OEDA office and the databases used by the organisation, in particular 'Spaidbas', a database mapping company information (employers) to their insurers. Includes

  • results of several queries dated October 1995
  • also instruction for using the fundraising table in a database referred to as 'OEDABASE'

The computer in question was donated by Rowley Ashworth Solicitors.

'Coda'

Correspondence with Rodney Nelson-Jones of Field Fisher Waterhouse, Solicitors, re their asbestos companies/insurers database Coda, its transfer to OEDA, updates to the database, etc. Includes
